Using the case study on the last slide in your learning materials, choose one of the models outlined in your reading and develop an outline of a plan to address the problem using your chosen model as a guide. 

This week we will investigate the various models to guide implementation of EBP.  Your text outlines several in chapter 13 (e.g. Johns Hopkins, Stetler) among which have commonalities.  Organizations who have implemented such models recognize the need for a systematic approach to practice change. As you begin to familiarize yourself with these models, contemplate how you might utilize one as a framework to implement EBP in the practice setting.  

CASE STUDY

Last Slide: For the last several months the rate of patient falls has been 2 times the national average in general and the rate of falls with injury has been 6 times the national average.  The facility has determined that a group needs to be convened to strategize how to address this issue.  Using your facility’s adopted model of EBP, develop a plan to address this issue.  Be sure to include all components of your model and be prepared to describe your plan.

Effective communication of new evidence is essential to evolving clinical practice.  The goal of dissemination is to facilitate the transfer and adoption of evidence into practice.  For discussion, provide a rationale for the method of dissemination that you think is the most effective in the transfer of information. What factors play into the method of dissemination chosen?  What considerations should be made prior to the creation of a dissemination plan?

The health facility is looking for an Evidenced-Based Practice that will help in reducing the number of fall injuries. The EBP model that would best solve the issue is the John Hopkins EBP Model. The facility chose this model because it is a helpful and practical approach to making appropriate clinical decisions (Dang & Dearholt, 2017).
