Introduction
Baccalaureate-prepared nurses have many opportunities to reflect on their contributions to patient care outcomes during clinical experiences. Research suggests that creating and sharing video reflections may enhance learning (Speed, Lucarelli, & Macaulay, 2018).
For this assessment, you’ll present your approved intervention to the patient, family, or group and reflect on various aspects of your capstone practicum experience. Such reflection will give you a chance to discuss elements of the project of which you are most proud and aspects of the experience that will help you grow in your personal practice and nursing career.

PART 1
Present your approved intervention to the patient, family, or group. Plan to spend at least 3 practicum hours exploring these aspects of the problem with the patient, family, or group. During this time, you may also consult with subject matter and industry experts of your choice. Be sure you\’ve logged all of your practicum hours in CORE ELMS.

Part 2
Record a 10–15 minute video reflection on your practicum experience, the development of your capstone project, and your personal and professional growth over the course of your RN-to-BSN program. A transcript of your video is not required.
You’re welcome to use any tools and software with which you are comfortable, but make sure you\’re able to submit the deliverable to your faculty. Capella offers Kaltura, a program that records audio and video. Refer to Using Kaltura for more information about this courseroom tool.

MODEL ANSWER

Good evening. My name is Jesmin Stringer and today I will present my Capstone project and then present my personal and professional development in the course of my RN to BSN journey.

1. Date of Meeting: Monday 7-2; Tuesday 6-9; Friday 8-1 and Sunday 6-12
2. The length of the meeting in hours: 12 hours in total
3. The meeting Location: Boardroom of the community center
4. The problem that was being addressed: The problem that the project has been addressing is hypertension affecting Mr. Smith, a 60-year-old diagnosed with the condition when he was 50 years and had since been using home-based remedial measures.
5. Why it was a problem for the Patient: Hypertension has significant risks, especially for elderly patients who are likely to suffer from other opportunistic conditions such as chronic conditions, falls, and degraded muscle strength. The greatest problem identified in my assessment is the patient’s lack of proper education and sensitization approaches to managing hypertension. Mr. Smith has not been guaranteed the best educational support and misses the best home-based hypertension management approaches.

• Intervention: The intervention developed for the case of Mr. Smith is educating the patient on the best healthcare management approaches like lifestyle changes and improving the collaboration with other made of the son and daughter were willing to work with me to help improve the patient’s hypertension condition that the patient s facing.
• How the family and patient will apply the intervention: After Mr. Smooth was diagnosed with hypertension, a health management plan was immediately drawn.
