Types of Diabetes
Diabetes is a global challenge that affects one-third of all hospitalized patients with a readmission rate of over 20 % from complications related to the disease (Mwathi, 2020). Diabetes can be divided into two main types: juvenile diabetes, type 1 which affects the youth, and diabetes mellitus, type 2 affects mostly adults. Juvenile diabetes also refers to as type 1 diabetes, in this type of diabetes, the body does not produce enough insulin due to a faulty immune system (Mwathi, 2020). Gestational diabetes also is a type of diabetes that affects certain women during pregnancy (Steiner, 2018). Some of the risks of developing diabetes include age 45, ethnic, racial background, history of gestational diabetes, hypertension, and hyperlipidemia (Quattrocchi et al., 2020).

Medication/Preparation
As per the latest guidelines, management of hyperlipidemia is considered while managing diabetes by adding a statin without assessing the risk of cardiovascular incident for that particular patient as it’s been proven to manage diabetes (Steiner, 2020). Diabetes organizations recommend the individualization of treatment goals and choices of drug therapy that will be safe and effective for the patients. Drugs like Metformin is considered the first line of drugs used for diabetes type 2 with A1c less than 7.5 unless contraindicated because it is effective and is a low-cost drug (Mwathi, 2020). Another aspect of the treatment includes the incorporation of lifestyle management such as medical nutrition, weight loss, exercise; diabetes self-management, and smoking cessation whenever appropriate. Most practitioners opt for 500 mg by mouth twice a day. The medication is taken twice a day by mouth preferably with breakfast and dinner (Quattrocchi et al., 2020). Some of the side effects of Metformin include abdominal discomfort, diarrhea, and renal insufficiency.
Short term/long term effects
Quattrocchi et al. (2020) report that short term effects of diabetes include hypoglycemia, hyperglycemia, skin issues, dehydration, blurred vision, and renal insufficiency. Long-term effects of the disease have been well documented in the form of retinopathy, neuropathy, macrovascular problems, worsening nephropathy, doubling of the serum creatinine level, and including death from renal disease.
Understanding how diabetes affects patients is vital in providing the necessary care needed to treat them. There have been studies that suggest some antidiabetic medications increase cardiovascular risks, while others claim to decrease cardiovascular risks. Most type 2 diabetes mellitus patients manage their illness with non-insulin therapies which usually lead to inadequate management or control of the disease. Diabetes mellitus is associated with an increase in cardiovascular disease (Quattrocchi et al., 2020).
Despite the advances in diabetes management, people with diabetes are not reaching their target glycemic goals and often require an increase in the dosages of their medications or an addition of another medication just to stay afloat. Healthcare professionals should be knowledgeable and equipped to decide on the best treatment regimen for each of their patients with type 2 diabetes or other forms of diabetes and be familiar with how to utilize the different medications available for better management of their conditions.
