Sonia Soto is an 18 year old woman who came to the U.S. from Colombia 1 year ago, speaks limited English, and has no family here. She has a 10^th grade education and does read and write in Spanish but very little English. Sonia had to drop out of school to work to support herself after her mother died from a drug overdose.  She says she was very sad for a very long  time then since she felt all alone, and had no energy to do anything except work and sleep. The father of this baby is not involved with her or the baby, and she struggles to earn a living cleaning houses. Her ex-boyfriend convinced her she did not need medical care for the pregnancy so she never saw a doctor throughout her pregnancy. He also got her involved in using methamphetamines and marijuana, which she was positive for when she came to the hospital 5 days ago.

Sonia has been  a patient in postpartum since she had her first baby , a boy Miguel 8pounds 3 oz, 3 days ago by primary cesarean section.  She came to the hospital 5 days ago with irregular contractions and was only 1 cm dilated but was admitted for induction because her AFI was only 5. Her drug screen on admission was positive for THC  although she stated she had not used for 1 month prior to hospitalization. She was in labor for 2 days but never dilated more than 2cm and her baby was having decels, so the doctor decided to do a c-section.

The doctor told Sonia’s nurse that he will probably send her home tomorrow, which means that as her nurse you need to be sure she has the teaching she needs to care for herself and her baby before discharge. When you walk into her room this morning you find her crying and ask her what is wrong. She says first of all she really does not feel well and thinks she has a fever. You take her morning vitals and they are T- 102.1, P-110, R-22, BP 110/76 and her abdominal incision is red with a small amount of yellow drainage. She also says she is very worried because she doesn’t feel she knows how to care for her baby, what’s normal, when his shots are due, when to go to or call the doctor, etc. She also is complaining that her breasts are really hard and painful when she feeds  Miguel and is asking if she is doing it right. Sonia is also feeling very overwhelmed and sad  and states she is a bad mom since she used drugs during the pregnancy, her baby is very yellow, and she is staying in a shelter right now since she has nowhere else to go or any help.
