Presenting Problem

The client is a 24 years-old African-American female. She currently lives with her white mother, African-American father, and 25-year-old African-American brother. The client was presented to the institution by her parents after a suicide attempt. The parents cited that she had displayed less confidence since she realized she was adopted, distanced herself from friends, and her school performance dropped (Hilsenroth, 2017). She started drinking excessively, which affected her studies and relationship with the family, she decided to quit school, and her father canceled her credit cards. She could not survive without the family, and she did not want to stay home. She resolved to overdose her mother’s pills but survived. She had never sought professional help before, but her parents had tried assuring her that she was equally loved despite being adopted.


Major Psychological Issues

The client experiences extremely low levels of self-esteem, as revealed during the first session. She believed that no one loves her and that she is not enough for her parents. She also claims that her friends do not value her and always abandon her when she has problems. She claims to be taken advantage of and receives nothing from others, especially those that she gets into relationships with.

Overview of Treatment Process

Sessions were held twice a week for the first four months. The sessions involved a combination of dialectical behavior therapy, schema-focused therapy, and family therapy. Although the client did not have a problem with anyone in the family, she always perceived that she was not as important to them as her brother because she was adopted. It was necessary to incorporate family therapy, which was aimed at improving communication with family members (Wheeler, 2014). More importantly, she needed reassurance that they care about her and that she was not less-worth for being adopted. The realization appeared to have a significant impact on improving her ability to communicate with her family. Family therapy was alternated with the other treatments every week to balance her progress.


The client’s main problem appeared to be low self-esteem and poor communication skills, which needed to be addressed by assuring her that she mattered to her family. Family therapy created a foundation for making sure that she does not hesitate to communicate her needs or preferences in any situation (Swift & Greenberg, 2015). She had always hidden her emotions from everybody, thinking that she was going to be a burden. Schema-focused therapy was directed towards making sure that she understands what she needs and establishes ways to meet them before they become a problem. Dialectical behavior therapy, on the other hand, was key in making sure that the client learns how to manage her emotions, develop an ability to tolerate stressful situations, and improve relationships.

Nature of Termination

After the first four months, the client’s sessions were changed to once a week, especially since she had displayed significant improvement. Family therapy was scheduled once every two months, or for the parents and psychologist to schedule if the client displayed the need for the session. Her parents were actively involved in the client’s treatment process as they would observe her progress and report any concerns to the psychologist. The client was highly cooperative because she allowed her parents to report any issues they felt needed to be addressed. However, her private sessions were kept confidential to avoid violating any HIPAA regulations (Swift et al., 2017). She was also allowed to discontinue therapy for two months, and her changes observed. Her parents reported that she did not show any problems. On resuming therapy, she agreed that she did not feel as she did before the treatment. Sessions were continued to assess her and make sure that she had indeed stabilized. It was established that she had improved and not showing any signs of deterioration (Hilsenroth, 2017). The termination decision was made because the client seems to have attained the desired wellbeing.


Goal Status

The key goal of the treatment was to address the confidence and esteem issues experienced by the client and improve her ability to communicate her needs with others. The first sessions were difficult, but the client showed significant improvement by the fourth month of treatment. The client’s self-esteem improved and she started to have a different perspective on life (Goode et al., 2015).
