Medical Diagnoses: Altered mental status due to sepsis
Mrs. O.M. is a 63-year-old Caucasian female was admitted for altered mental status and sepsis. She was brought to the emergency department by her daughter for progressive weakness, change in mental status and confusion. Upon arrival to the ED patient was noted to be hyper-somnolent but arousable. She was oriented to place, self, and at times to time, no slurred speech, able to wake up and start conversation but drifts back to sleep easily. Patient has history of seizures, multiple CVAs, diabetes mellitus, hypertension, hypothyroidism, anemia, anxiety, depression, chronic back pain, and fibromyalgia. However, at this time she presents evidence of decrease functionality. Her daughter denied fever, cough, shortness of breath, respiratory symptoms, nausea/vomiting, headaches, or projectile vomiting. She is on the hypertensive side with pressures in the 170s systolic, she is sating in the high 90s on low flow oxygen.
Primary Medical History of seizures, multiple CVAs, diabetes mellitus, hypertension, hyperlipidemia, anemia, anxiety, depression, chronic back pain, hypothyroidism, and fibromyalgia.
Labs indicated:
Lab test significant leukocytosis 20K, urinalysis consistent with UTI, metabolic acidosis, and hyperglycemia
