Cultural competence for healthcare professionals is defined as ‘the ability of healthcare providers to effectively deliver healthcare that meets the social, cultural, and linguistic needs of their patients’ (Harrison, Walton, Chauhan, Manias, Chitkara, Latanik, & Leone, 2019). I will be looking at BK, a 16-year-old Caucasian female who came in with her grandmother, The grandmother reports that BK has been acting strange, always staying in her room and refusing to do all the things she used to enjoy like singing in the youth choir. Grandmother also shares that BK’S mother is a drug addict that is currently incarcerated. The grandmother is worried that BK is using drugs and wants her tested for drugs. BK is not on any medications. She is obese and does not make eye contact when you enter the room.

BK appears to me that she is depressed., this may be partly the reason she is refusing to leave her room and stop doing all that she likes to do. Being obese and not having her mother in her life may also be stressful and affect her negatively. Difficulties in regulating evoked emotions after stressful life events are considered to lead to depression (Stikkelbroek, Bodden, Kleinjan, Reijnders, van Baar, 2016). She may be feeling abandoned and inadequate, giving her low self-esteem.

When assessing adolescents, it is critical to ask all social history questions in a nonjudgmental way to elicit potentially important historical elements that can add understanding to the etiology of otherwise common symptoms or diagnoses (Silverstein, Zander, & Middleman, 2020). I will respect BK’s space, start talking to her on a general topic to engage her and make her relax. Before asking her personal question (s) I would ask if she wanted her grandmother present or not, thereby respecting her privacy and building a patient -provider relationship. The five targeted questions that I would ask concerning her health history and to assess her health risks are:
1. What do you and your friend do for fun?
2. Do you like sport? If yes, what sport?
3. What kind of food do you like? How often do you eat?
4. Do you feel safe at home?
5. Do you have any thoughts of harming yourself?
I envisage BK not wanting to open to me and probably not answer my questions at the first visit. I am not certain if she will agree to the drug test requested by her grandmother either.
