Cultural Competence
As an advanced practice nurse, understanding socioeconomic, cultural factors, spiritual preferences, and patient lifestyle choices are essential for appropriately treating patients since these factors affect one\’s health (Ball et al., 2019). APRNs must be culturally competent to take care of their patients. Cultural competence is interacting and understanding people from different cultures (Ball et al., 2019). Cultural competence acquires obtaining a set of behaviors applied to caring for a diverse group of patients in one’s career (CDC, 2020).
Explanation of the specific socioeconomic, spiritual, lifestyle, and other cultural factors associated with the patient you were assigned.
In the assigned case study for this week, the APRN can see that the patient is in her teens. She was living with her grandmother and is obese on no medications. The patient’s mom is a drug addict incarcerated and recently has not enjoyed activities she used to enjoy, such as singing in the youth choir. Knowing that the patient has been acting strange and not doing things she wanted should alert the APRN that she may be depressed. Adolescents who have parents in jail are at elevated risk for mental health issues (Davis & Shlafer, 2017). Depression is also associated with teens who experience stressful life events, drug abuse, female gender, and lack of social support (Anosike et al., 2020).
As a teen girl, having the help of her mother is essential. BK could have been close to her mother before her incarceration. Not having her mother as a support person at this age could be hard on her. Change in family structure can also play a role in shifting the patient\’s attitude (Davis & Shlafer, 2017). Grandparents caring for adolescents often have fewer resources, multiple needs, and health problems affecting one\’s relationship (Davis & Shlafer, 2017). The patient also seems to have religious views or is exposed by being in the youth choir. Sigurvinsdottir et al. (2021) state that religious participation did not relate to depression. It cannot be assumed that because the patient may have religious values that she would not be depressed. Many adolescents find comfort in their family\’s cultural traditions (Ball et al., 2019). Assessing why BK does not attend choir anymore may open an opportunity to explore this cultural characteristic of the patient.
Explain the issues that you would need to be sensitive to when interacting with the patient and why.
Issues the APRN would need to be sensitive to are the fact that BK is obese. Young girls in this age group face multiple pressure to fit in. More often than with boys, teenage girls face challenges with body image through peer pressure, social media, and high sexual harassment in schools across America (Nemiary et al., 2021). BK could be struggling with self-image leading to her behaviors. Stressful life events, teasing of pears place teenagers at higher risk of depression (Meniary et al., 2021).

Sensitivity would be required when discussing the home environment, being that her mom is incarcerated. Discussing BK\’s situation with her mother may be a challenging topic to discuss with her. The patient is experiencing internal conflict that could be coming from living with her grandmother, her mother is incarcerated, and exposure to drug abuse at such a young age. APRNs must learn to be culturally sensitive to socioeconomic, religious, lifestyle choices that could be affecting BKs health.

Obtaining an accurate health history from adolescents may be challenging. Confidentiality is essentially vital to young adults (Ball et al., 2019). Trust is also an essential factor in the provider-patient relationship for this age group (Ball et al., 2019). In this age group, giving information in the presence of a family member may be difficult. Asking the grandmother to step out during the interview process will aid in obtaining information from BK.
