Examples for internal communication: Staff meetings, town halls, intranet AND Examples for external communication: billboards, Facebook, Open houses, direct mailers.

Question: is there a required number of sources to be utilized?
Answer: There are four required citations. All four are websites. Please have the Writing Center check your in-text citations and Reference page proactively. The information from B5 thru the end of the paper is based on your expert opinion from working with your patient population, in your organization, and in your community.

Section E1: Stakeholder Roles and Responsibilities
Question: How many stakeholders do I need?
Answer: We recommend 4 to 6 stakeholders. You will need to continue to use this same group of stakeholders in sections E2 and F1.
Question: What does it mean by stakeholder responsibilities?
Answer: In section E1, you will discuss the stakeholders’ roles and responsibilities as it pertains to your plan. For example, if your goal is to decrease noise and nursing is a stakeholder, you would describe that nursing will be assessing the patient to determine if interventions are needed, providing a quiet environment, and monitoring the noise and intervening if necessary.
Section E2: Stakeholder Accountability and Involvement
Question: On E2 what would be accountability for a patient? I got the involvement; the only thing I can think of would be improved health?
Answer: Accountability for a patient could be ensuring they are providing feedback during their visit. They could also be accountable for completing the survey. You could state the patient is accountable for being involved in their care by actively participating so they could achieve improved health.
Question: For section E2, how do you differentiate between SH accountability and involvement and how is involvement different from the previous section’s responsibilities?
Answer: In section E2, you will discuss how to ensure stakeholder accountability and involvement. An example of how to ensure accountability is sharing the expectations with the stakeholders and holding them accountable if they do not follow the new policy or guideline that you created in your plan. An example of how to ensure stakeholder involvement is to conduct a survey on the best times to meet to discuss the new plan so that the stakeholders can be involved.
Section E3: Training
Question: What needs included in training?
Answer: In this section, you will discuss the training that is required for your plan to be successfully implemented. You will primary be looking at sections D1 and D2 to see your organizational change and your processes that make up your plan. For example, if your organizational change were to implement a policy for a “No Pass zone” to improve responsiveness, you would discuss how you would train the staff on the “No Pass zone” policy.
Question: Do I have to include all three questions in the training?
Answer: Yes, you need to discuss the training for each of the three questions that make up your plan. You may do this as a combined plan or as three separate plans.
Section E4: Plan Implementation
Question: Is E4 on one of the goals or all three?
Answer: In E4, you will make a timeline to discuss how you will implement your plan. Yes, you will discuss each of the three organization changes in this section. You may do this as a combined plan or as three separate plans. Also, make sure you include periodic review checkpoints to measure progress. Examples of periodic review checkpoints could be review of your HCAHPS data, review of your call light response time, and review of documentation of medication or discharge planning education.

Section D1: Organizational Change
Question: Section D1 asks us to explain how change can help improve scores. Do I talk about change in general for each HCAHPS question or the specific changes I plan to make?
Answer: In section D1, you will choose one organizational change for each of the three questions you chose in section C. A few examples of an organizational change could be a new policy or implementation of a new technology to aid in improving your HCAHPS scores. You will then discuss how each organizational change will improve your HCAHPS scores.
Section D2: Structure, Process, and Outcomes
Question: Can section D2 be completed as a table as demonstrated in the cohort? The task says to provide a “logical discussion” which implies paragraph form to me.
Answer: Yes, section D2 can be displayed as a table, but you would need to ensure you are providing enough information to meet the requirements of the rubric. If you do a table and then provide narrative to ensure you have provided a “logical discussion” in this section. For example, in the table you can have bulleted points on your structure, process, and outcome. Be sure to include evidence-based practice, shared governance and technology in Process. The Course Instructors recommend that the students include all three for each question.
Section D3: Improving Organizational Quality
Question: Do we need to have EBP literature for D3 or just discuss how we are to incorporate it?
Answer: You just have to describe how you would incorporate it (and shared governance too) and how it would improve organizational quality. Do not forget about the improvement to organizational quality.
Section D4: Shared Accountability
Question: Can you explain how I would discuss how payers are accountable?
Answer: In this section, you are providing a discussion of the methods you will use to ensure the payers (and the other 3 groups – patients, providers, and personnel) are accountable in your plan. For payers, you could discuss that you could collaborate with the Finance department to ensure that the payer is going to provide reimbursement (which will improve financial stability) and the payer will provide feedback to the organization on any concerns (which will improve the plan by allowing leadership to make any necessary changes). The payers are accountable to provide education (on diagnoses, medications, how to advocate for yourself, etc.) either via a website or snail mail.
Section D5: Technology Trends
Question: For section D5, do I need to discuss technology in general or do I need to include it for all of my new processes?
Answer: You need to discuss how you are incorporating technology in your overall plan. You do not have to give an example for each of the 3 HCAHPS questions you are addressing.
Section D6: Improve Care Delivery System
Question: I am having difficulty with section D6 concerning improving the care delivery system.
Answer: The section is asking you to discuss how your plan improves quality in your organization. How does your plan improve cost for your organization? How does your plan improve access for patient care in your organization? How does your plan demonstrate the patient being at the center of care? This is four different paragraphs
Question: Can someone help me understand what is meant by “improved Access” in section D6?
Answer: Section D6 is asking you to discuss how the methods in your plan (your processes) will improve cost, quality, access and patient-centered care.
Examples for access: If nurses communicate more with patients, their access to care improves. The more they interact, the more information is shared, and care is improved.
If you improve responsiveness by implementing a “No pass zone”, your patients will have improved access to the staff. Improved responsiveness will improve quality by decreasing falls.
Section D7: Improve Financial Stability
Question: We already discussed financial impact. How is this section different?
Answer: In this section, you are discussing how the methods in your plan will improve financial stability. Discuss how improving the HCAHPS scores will improve reimbursement.

Section B6: Financial Impact
Question: It says to explain potential short and long term financial impact on the organization. I take it that this is referring to HCAHPS scores and how they affect the organization. Is this correct?
Answer: Yes, in section B6, you are discussing the potential short and long-term financial impact that HCAHPS can have on your organization. What will happen in your organization if your HCAHPS scores are lower than the other hospitals around you? What if they are lower than the state and national average? You need to address both short-term and long-term impacts in this section.
Section B6a: Impact on Quality
Question: Do I need to address each HCAHPS question when answering this section?
Answer: No, but you do need to give some examples on how the decreased reimbursement as a result of lower HCAHPS questions can potentially impact quality outcomes. For example, you could discuss how low responsiveness scores could potentially impact quality with patients having higher fall rates as they are trying to get to the bathroom on their own because staff has not responded timely to the call light.
Section C: Cause of Scores
Question: Can section C be answered in a table?
Answer: Yes, a table can be used for the causes of the scores. Just remember, you still need to provide narrative as the rubric states you need to “provide a logical discussion” with support of the potential causes of the HCAHPS scores. In this section, you will need to identify three (3) HCAHPS questions and then identify (3) potential causes for each. We recommend listing at least 3 potential causes for each question.

Section B4: Demographics and Services Provided
Question: What demographics do I need to share?
Answer: You need to share the cultural (race, values, beliefs, religion, language), educational (high school, college completion rates), and socioeconomic (unemployment rate, annual income) dynamics for your community. This information will be used to answer the next 3 sections so you need to include all 3 areas.
Question: Do I need to discuss all the services my hospital offers?
Answer: No, you do not have to include every service your organization offers. You need to include a comprehensive list and then provide a narrative describing the services.
Section B5a, B5b, and B5c: B5a – Cultural Dynamics, B5b – Educational Dynamics, B5c – Socioeconomic Dynamics
Question: (specific to B5c) I am having a hard time thinking of how the demographics in the area of my chosen facility could affect HCAHP scores. The area is affluent with the highest percentage of doctoral degrees in the state. I am trying to figure out how this could have a negative effect on HCAHP scores.
Answer: (specific to B5c) Patients of higher or lower socio-economic status may have different expectations and therefore may not be satisfied with the care they receive. You will need to write about the socioeconomic dynamic (ex. Affluent population) and what HCAHPS question it could influence (ex. Responsiveness).
In sections B5a, B5b, B5c, you must take your demographic data and discuss HOW it could influence your HCAHPS scores. You want to be specific and discuss the HCAHPS question(s) it can influence. For example, if you have an affluent population, this can potentially decrease the responsiveness HCAHPS questions as your population may have higher expectations of response time than what the staff can deliver. You only need to discuss the potential impact. You do not need to solve anything in this section. You will do that later in the task.

Question: For Section A, do we post HCAHPS scores for just our medical facility of choice or also comparison scores of 2 other facilities?
Answer: In section A, you just need to share your hospital’s HCAHPS scores. In B1, you will share the HCAHPS data (a variety of HCAHPS questions) for your hospital, along with your state and national data. You will also need to write a narrative on how your hospital compares on the HCAHPS scores to the state and national average. In B2, you will share the HCAHPS data for your hospital, along with the HCAHPS data from two other hospitals. You will also need to write a narrative on how your hospital’s HCAHPS data compares to the other two hospitals.
Question: Do we have to explain the table in section a and b (1,2,3).
Answer: You will need to provide a comparison for the scores in sections B1, B2, and B3. You need to discuss how your scores rate in comparison to the state and national average (B1), two other hospitals (B2), and the rate and number of surveys (B3). You cannot just display the scores, as you would not be comparing the information.
Question: I am having trouble copying/pasting my tables from the website – It never comes out right. Any pointers? Do I have the option of creating my own table with the information on it?
Answer: One tip is do NOT include the star ratings. They can mess up the formatting of the table.
